Membership entitles patient(s) listed in application unlimited adjustments priced at $30 per member per adjustment. If family membership is selected, additional members may be added to membership at any time with no change to monthly membership fee, up to 4 members total.

New patient exam fees are not included in membership.

Membership pricing is valid for 12 months at which a new membership application must be completed with updated billing information. Membership fees are subject to change at renewal. Adjustment fees are subject to change at clinic's discretion.

By signing this application, you (or guardian, as applicable) authorize Creer Chiropractic to charge the card you have specified above. Membership fees will be withdrawn on the 1st of each month set up as a recurring payment. Additionally, your signature authorizes Creer Chiropractic to charge the credit card on file in lieu of presenting it for any services received at your request.

Member is responsible for updating Creer Chiropractic with any changes in billing information including, but not limited to, change of payment method, expiration date, and patient billing address.

Membership may be cancelled at any time. Membership will not be prorated and member(s) may continue to receive membership pricing for adjustments until the end of the month at which time membership ends and recurring payments will discontinue.

If membership is cancelled before first billing cycle, any adjustments received at member pricing are subject to full non-member rates and any difference will be collected at time of cancellation.
